Kindergarten Requirements For All Students

Including The New Vision Exam Requirement

ATTENTION PARENTS OF STUDENTS ENTERING KINDERGARTEN

NOTIFICATION OF 3 REQUIREMENTS THAT MUST BE MET

 

 

  1. The state of Illinois has passed a new Public Act that requires all Kindergarten students to have any eye examination by October 15th by an optometrist or a physician who provides eye examinations.  A blank form is available on this web site under the heading "Vision Examination Form".

Below is the official statement:

“Vision Examinations:  Public Act 95-671, effective January 1, 2008, requires that all children enrolling in kindergarten in a public, private or parochial school and any student enrolling for the first time in a public, private or parochial school shall have an eye examination.  Each such child is to present proof of having been examined by a physician licensed to practice medicine in all its branches or a licensed optometrist within the previous year before October 15 of the school year optometrist.  Moreover, parents or legal guardians who object to eye examinations on religious grounds shall not be required to submit their children to the examination if such parents or legal guardians present to the appropriate local school authority a signed statement of objection, detailing the grounds for the objection.  Each public, private and parochial school is required to give notice of this eye examination requirement to the parents and guardians of students”.

 

2.  All students entering Kindergarten must have a physical examination and updated immunizations before starting school.  The physical needs to be within one year of the start of school and must include the required 5-year-old shots.  This is an Illinois state requirement. Your child will not be able to start school if this is not done and will not be allowed to attend until this requirement is met.  Please bring physical exams and shot records to central registration on August 7th or 8th, if you have not turned them in already.

 

3.  All students in Kindergarten are required to have a dental exam by May 15th, 2009.  This is an Illinois state requirement. 

 

The school district nurses will be available for questions at Central Registration on August 7th & 8th, 2008.
