Requirements for Entering Kindergarten

for all students

3 REQUIREMENTS THAT MUST BE MET

1. All students entering Kindergarten must have a physical examination and updated immunizations before starting school.  The physical needs to be within one year of the start of school and must include the mandated 5-year-old shots.  This is an Illinois state requirement. Your child will not be able to start school if this is not done and will not be allowed to attend until this requirement is met.  Please bring completed physical exam forms and shot records in as soon as possible to the nurse. The blank form is available on this web site under the heading "Physical Examination Form". See your school nurse if you need help finding a health care provider.

2. The state of Illinois has passed a new Public Act that requires all Kindergarten students to have an eye examination by October 15 of the Kindergarten school year by an optometrist or a physician who provides eye examinations.  A blank form is available on this web site under the heading "Vision Examination Form". WalMart in Glen Carbon & Crown Vision Center in Alton will accept Medicaid and All Kids.

3.  All students in Kindergarten are required to have a dental exam by May 15 of the Kindergarten school year. This is an Illinois state requirement. A blank form is available on this web site under the heading "Dental Examination Form". See your school nurse if you need assistance finding a dentist.
